ADMISSION REQUIREMENTS AND PROCEDURES


HIGH SCHOOL DEPARTMENT

Admission requirements are presented to the Admission Committee composed of the Principal, the Registrar, the CCF Coordinator, and the Guidance Counselor. As a general rule, a student is officially enrolled if he has submitted all his credentials and upon payment of tuition.

First Year Applicants:

  • Original Report Card (Form 138) duly signed by the Principal, preferably with a general average of 85% with no grade below 80% in any subject.
  • Entrance test result
  • Birth Certificate
  • Catholic Baptismal Certificate
  • Marriage Contract of parents
  • ID pictures
  • Certificate of good moral character from the school last attended duly signed by the Principal
  • Interview by the Admission Committee

Transferees

  • Same as first year applicant
  • If transfer is done during the year, current Form 138 and Certificate of Transfer Eligibility duly signed by the Principal of the school where the student comes from
Old Students
  • Original Report Card (Form 138)
  • Summer Card (For Summerians)

Foreign Students

  • Certificate Proof of compliance with the Commission on Immigration requirements and the Department of Foreign Affairs
  • Certificate of Eligibility for Admission from the Department of Education
  • Photocopy of Alien Certificate of Registration (ACR) against the original and signed by the Officer from the Foreign Division. Department of Education
  • Report Card (Consider Department of Education Guidelines for Determining Grade/Year Placement of Students from Foreign Countries Seeking Admission to Philippine Schools)
  • Birth and Baptismal Certificate (if available)
  • Entrance Test Result
  • Interview of parent/guardian by the Principal

HIGH SCHOOL - NIGHT SCHOOL

Requirements and Qualification
  • The applicant must at least be 18 years old upon enrolment and a graduate of the elementary course.
  • S/he must be an out-of-school youth or working.
  • S/he must come from indigent families.
  • S/he must have the recommendation of his parish priest or the area leader of the parish to which s/he belongs.
  • S/he must submit the following upon enrolment:
  • Certification/Form 138/Form 137 showing completion of the elementary course or a level
  • Birth certificate
  • Picture               
  • P300 registration fee

HIGH SCHOOL - ADMISSION PROCEDURE

A first year student who wants to apply for admission does the following:
    • Gets an application form for admission from the Guidance Center.
    • Submits all the credentials with the accomplished application form (Form 5) to the center.
    • Underqoes an interview with his/her parents/guardian.
    • Pays the entrance fee at the Cashier's Office.
    • Presents the official receipt to the Guidance staff who will schedule the applicant for the entrance test.
    • Takes the entrance test on the scheduled date.
    • Once he qualifies, pays to the Cashier's Office the reservation fee.
    • Enrolls on the scheduled date.
EARLY CHILDHOOD EDUCATION

Age Requirements

A. Toddler - at least 2 to 2 1/2 years old at the opening of the school year
B. Nursery - at least 3 to 3 1/2 years old at the opening of the school year
C. Kindergarten - at least 4 to 4 1/2 years old at the opening of the school year
D. Preparatory - at least 5 to 5 1/2 years old at the opening of the school year

Other Requirements

A. Birth Certificate
B. Parent's Catholic Marriage Contract
C. Catholic Baptismal Certificate
D. Three (1" x 1") I.D. Picture

Procedure for Incoming Grade One and Preschool Pupil

I. Requirements

A. Age

Toddler - must be 2 to 2 1/2 years old at the opening of the school year
Nursery - must be 3 to 3 1/2 years old at the opening of the school year
Kindergarten - must be 4 to 4 1/2 years old at the opening of the school year
Preparatory - must be 5 to 5 1/2 years old at the opening of the school year
Grade One - must be 6 to 6 1/2 years old
Note: Age state by June 2007

B. Documents

Original and photo copies of:

  • Birth Certificate from the City Hall
  • Roman Catholic Baptismal Certificate
  • Parents' Catholic Marriage Certificate
  • Two (2) 1 x 1 recent pictures - (colored or black and white)
C. Screening Fee: Not Refundable

II. Instruction

  • Fill up the application form (Form 5)
  • Secure the necessary paper requirements (Letter B).
  • Submit the requirements and PCS FORM 5 personally either by the mother or father at the Guidance Office.
  • The schedule and the permit for the examination will be given after submission of requirements
Please be informed that:
  • The grade one applicant must pass the oral test in order to qualify for the written examination.
  • The School has the right to replace your child if he/she is unable to come on the scheduled testing day.
  • Original copies of birth and baptismal certificate and Parents' Marriage Contract will be used only to verify the photo copies. These will be returned to the applicants after verification.

Monday to Friday        8:30 AM - 11:00 AM
                                   1:30 PM -  3:00 PM
Saturday                     8:00 AM - 11:00 AM
